His powers have changed so much between his introduction and now that "steel-hard organs" wouldn't honestly be beyond him.

It's sort of backed up by canon. Christopher Priest had Cap and Falcon track down Luke once to get a blood sample, using a Vibranium needle, because his blood was still infused with the chemical substance that had given him his powers in the first place. If Luke is still full of chemicals, it's probably not beyond the bounds of comic-book reason that he's growing steadily tougher.

I thought his interior had the usual kind of conditions - in Secret War, he gets blown up and while his skin isn't pierced, is interior is battered about quite seriously - then they can't operate, because they can't get through his skin.
