It's from a special What If that showed various versions of what would happen if Reed and Sue's second child had lived without the aid of cosmic retcons. This particular one became some sort of life-sucking eldritch horror.

Computo killed one of Triplicate Girls bodies... her reaction to which was NOT "Oh god, oh god it killed me", but "Oh well, I'll now change my name to Duo Damsel instead". The SilverAge wasn't much on traumatic shock.

I don't remember the exact issue but it was one of those short stories they used to print in Marvel Adventures, after they stopped printing Mini Marvels. It's basically 3 or 4 panels and the art is really bad. Oh and it's neither funny nor cute.
