It's about trying to save their dying grandmother, so I wasn't expecting butterflies and unicorns at the outset. Plus I think it sets the scene nicely for what's to come.

Of course, the nurse has to be someone major, perhaps Loki setting up the pieces for mischief, or one of the embodiments of Death, there to show that death isn't always scary when Granny's time comes, which I'm pretty sure it will.
