Maybe, maybe not. After the Red Skull's consciousness was transferred into a clone of Steve Rogers' body and his face was re-skullified due to inhaling his own disfiguring gas (a pretty obvious rip-off of the Joker venom), the rest of his body was still pretty normal. This isn't the same thing, but still.

(It does make me wonder if this version of the Skull-stuff is supposed to be some sort of super-soldier serum derivative, because even though Doug-skull doesn't have a Steve physique, he's a lot more athletic than I remember Doug being.)
