It's how she was introduced in the 1960's, as a member of the Frightful Four. It predates even the debut of the Inhumans.

Medusa grew up with the other Inhumans, but when their servant class, the Alpha Primitives, rebelled, she took part in the fight against them, and her aircraft went MIA due to damage caused in battle.

She survived, but was amnesiac and eventually found by the Wizard, who recruited her and she became "Madam Medusa".

The notoriety she gained, drew the attention of the Inhuman Royal family, who came to find her, resulting in her regaining her memory,

Hey, don't take it personally. Nobody really needs to know who the Wizard is, but it's kind of amazing how quickly the common frame of reference changes in comics. Hell, people get nostalgic for Young Justice or the Busiek Avengers nowadays.
Besides, I-feel-old is kind of a theme in my life recently.

I think the first time might actually also be the last time. Medusa never joined again as far as I know and Sandman got shuffled off to the Spidey rogues soon after.
Nowadays the Frightful Four are usually Hydro-Man, The Wizard and whoever else he can grab off the street at any given moment.

Sandman - "Oh you know, went good through well developed character plotline, became a reserve Avenger for a while, then went bad again because Byrne was in a mood and had you revillainise me"

Wizard - "Oh yeah, sorry about that. Pete?"

Pete - "TRAPSTER!!! Dammit, I'm the TRAPSTER!!!"

Wizard - "Still on that kick, eh Pete? Medusa? How's that prehensile hair serving you in your career path"

Medusa - "Me? I'm now Queen of the Inhuman/Kree Empire spanning much of the Greater Magellanic Cloud and beyond."
