According to the back-up story that ran across a bunch of different comics Vision was originally basically an unmanned satellite launched as warning message and was rebuilt by the people of the world it crashed on to mimic the form of whatever race she encountered.

But with, y'know, tits. Maybe a long chin if required.

She had sort-of clothes originally, presumably they didn't last the journey.
