I gotta say, the main character's innocent, simplistic frame of mind somehow makes the horrible things she does even worse -- and yet, kinda weirdly absolves her a little of them? She honestly thinks she's doing the right and ethical thing, and she's incapable of understanding why she might be wrong. That's a pretty good hook for a character!
As with all things in this series, it varies ^^; One of the chapters skipped between this and the last-posted one has a plot that's very very difficult to argue doesn't hinge on Fran understanding full well her surgeries are a penalty worse than death.
