This is actually a nightmare Doreen is having, caused by (you'd never guess) Nightmare. So Nefaria's state of being dead or alive is irrelevant, as emphasized when she goes up against Doctor Octopus earlier in the issue.

This is also why he challenges her to something involving counting, as she knows absolutely nothing about him without her Deadpool Cards, so she assumed he had the name Count Nefaria because he loved to count things. And because it's her dream, she's right!
