One does wonder whether anybody even noticed that Iron Man and Mr. Fantastic had been replaced by their doppelgangers.

Yeah, Wolverine noticed that they were fakes as soon as he entered the room. Daredevil confirmed it when he sensed the gamma bomb and started the mad dash for the podium.

I have the main issue, and yet only now did I realize that it broke down into superheroes vs. mutant.

Repli-Reed and Phony Stark deliberately agitated it by declaring that evil doppelgangers were "probably another mutant menace." Though it was still very stupid of everyone to start throwing punches.
