Teen Tony isn't dead, he's just badly injured in a way very similar to Tony's original problem with chest shrapnel.
Then Onslaught happens and he does die, then thanks to Franklin Richards is merged with Adult Tony, who is so embarrassed by the whole thing (not the part where he turns evil and murderous) that he'd rather no-one ever speak of it again.
