Much to my own surprise, I don't think I've seen that posited as a story idea before... though I hope it was before that world's equivalent of "A Death in the Family", because otherwise who new levels of horror.

There's an Elseworlds story called Batman: Two Faces that does something similar, basing itself on a certain Robert Louis Stevenson work.

Two Face figures heavily, but it is actually Joker and Batman sharing a body. Bruce Wayne attempts to create a cure for Two Face's split personality, but ends up exposing himself to it and manifesting the Joker personality.

As "The Joker," this Bruce Wayne went around saving innocent people while delusionally believing he was killing them.
