I was gonna say no, he visited Good ol' Ben Kenobi's cabin in Gillen's 2015 run... buuuut I decided to double check and... yeah, he already visited this place with Aphra searching for information on Luke so... yeah, I guess we are officially at the point the comic continuity is contradicting itself. (Altough I will be fair and say this issue gave more emotional weight to Vader visiting the Lars Homestead than in Gillen's run)

Pretty sure it's Sabe, the body double played by a then-unknown Kiera Knightley in Episode I.

Which answers the trivia question, yes she and Portman have worked together and even used their stunning resemblance for the role.

When first learning the Rebel pilot that blew up the Death Star was named Luke Skywalker, Vader could have still not known the whole truth. "Kenobi found and trained a Force-sensitive child. And he gave the boy last name of Skywalker? Is that Kenobi's idea of a JOKE?!"

Another 'funny' thing Vader could have realized before Empire Strikes Back:
"Note to self, keep this information secret from the Emperor. Second note to self, act surprised if the Emperor finds out on his own and then tells you."

I always had a theory that "Skywalker" was just some common, intuitive trade-based surname like "Cooper", so there's like, a whole lotta "Skywakers" on a moisture farming planet. I like to think RISE OF SKYWALKER validated this theory.
