The main story has Batman as one - his struggle with the azmer the Orgham family'd inflicted on him's ripening him up for Barbatos.
In issue #1081, Talia al Ghul'd set him on a path to overcoming the azmer - had taken him to the Aras desert for it.
At the outset of Batman's subconscious struggle, Doctor Hurt'd appeared and left.
When Batman's progress took him to " the Gotham that the Batman saved ", the doctor came again.
" This place.. it isn't real. "

Batman lunged at Hurt - they fell from their height to the ground, amongst others.

The clown climbing the money played the Joker and scored a point off Batman that followed from Hurt.
It amused the doctor: " Even in an illusion you cannot envision for yourself a victory. "

Hurt left - and so did Batman, one hypothetical Gotham behind him.
Back in the real Gotham, the Question followed her own course.
Her investigation into the death of GCPD detective Henry Fielding - whom she'd ordered into danger, as police commissioner Renee Montoya - brought her up against the Orgham family's work in Gotham.

(This is the Orghams continuing " the process of cleaning [the] festering wound[s] " of Gotham.)
Renee listened to Gotham try to remember what the Orghams' Reality Engine'd made her forget.
' And sometimes, the mere memory of him is enough..

Batgirl went through the Orgham men.
The Question stopped her on the last one: " I need him to talk. "
Back in the Aras, Batman met Doctor Hurt again in " the Gotham that the Batman doomed. "

Batman beheld himself bolstered by compensation.

Back in Gotham, Batgirl and the Question's captive fell.

' It terrifies him. '
The Orgham man hung from Batgirl's line scared - the Question interrogated him.
Her angle was Fielding's partner Nash, whom she'd found dead - who'd had a burner phone on him.

" Gotham says the calls were made to you.
" Why? "
(This is Renee Montoya explicitly doing " Question as urban shaman " stuff, sold well by the art and panel composition.)
The Orgham man answered that he'd been the one to bring Nash " into the fold " - that Nash was " working for us.. f-for the Orgham lady. "
(He's describing plot from #1075 - but not completely enough to give Renee the answer she wants.)
Batgirl and the Question let their captive go, into a dumpster.

Batgirl's parting remark left Renee thinking about herself " before everything " - about being a detective, pursuing the " faceless truth ".

Off she went, on her lead to the Orghams.

(That's the face-eating killer Eduardo Flamingo - in #1080, Doctor Hurt took him along to handle Talia.)
The backup story continues from #1081 - the used and discarded subjects of Doctor Hurt there who'd sworn vengeance on him had found their way to a place where they could take it.

Inside, the Doctor Hurt Revenge Society found that Hurt hadn't even been original in hurting them.
There was Annie, whom Simon Hurt'd married and left - she met " one of his brides ":

" Would you like a drink? You look like you need a drink. "
There was Raoul, who'd wanted to be a superhero - who'd worked under " Master Hurt "'s guidance, who'd ended up jumping off a building with a grapple gun that didn't work.
He found his way to Hurt's " Übermensch Spectacular ", whose enthusiastic emcee showed the eager audience attractive and accomplished hard workers dissolving in something acidic:

The revelry appalled the revengers.

Raoul opted for immediate revenge - he had a gun he'd brought along, and the revelers were provocation.

They ran from gunshots - they found Annie's fellow bride of Hurt poisoned, amongst others.
" H-hello. You're a little late. "

The poison in the wine accounted for Annie, leaving just two who wanted Doctor Hurt dead - James and Selma.

" We can do this. You and me. "

Hurt is, as he claims in R.I.P., 'the piece that does not fit.' He had multiple origins in his initial appearances, and connections with Darkseid. Dan Watters has a great grasp on the character imo.. makes me want to check this run out in its entirety. Ty for posting!
