The preview this is from doesn't really give context... But from the last couple issues...

Joker has come back into Harley's life and wants her to come back to him. She didn't. In fact, she followed up on the threat to slaughter him if he showed up in her life again that she made last time she saw him. She dropped him into a crossroad, tied up and wearing a 'Brooklyn Sucks' sign...after he's knocked around (a lot) by drivers not even trying to avoid hitting him, he's alive, but unconscious. This appears to be his fantasy while his lights are out.

Wow, those drivers must've really done a number on his head if that's his fantasy of What Could've Been with Harley. I'd have expected something more like her fantasy of settling down with the Joker, from Mad Love.

Somehow, out of all of that (with the exception of the implicit normalisation of a grossly abusive relationship) the most horible thing about that image are the hanged bodies of Babs and Buster Bunny (no relation) from "Tiny Toon Adventures"
