Okay, time to bring out the heavy stuff here. This tale of revenge upon revenge is so ridiculously, over-the-top grotesque and brutal it makes EC's "Foul Play" look like The Very Hungry Caterpillar by comparison. Warning for violence/gore.

Did the surgery disfigure him or not? He looks just as bad as he did before.

That was my thought exactly. It'd make more sense (to the extent anything he does makes sense) for Hugo to swear revenge on the doctor for failing to make him look any better, not for making him look worse.
