Fisk considers clones to be abominations. When Ben presented him a clone of Vanessa he viewed it as a desecration of her memory.

it seems Bendis original plan for "Spider-Men II" before the ultimate universe had been destroyed involved 616!Miles trying to reach the ultimate universe in an attempt to find a parallel version ofhis dead wife.

So bendis seemingly forgot (or decided to ignore) that the ultimate universe is no more and did his original plan anyways.

Is the Ultimate Universe still gone?
Shouldn't that have been one of the realities that Reed and the Future Foundation recreated?

Well, they don't explicitly say that they're going to 'the' ultimate universe. It could be that the Richards family created a new ultimate universe in the new multiverse, but Miles and the Maker are still the only survivors of the original, 'real' ultimate universe.

Actually, that would be pretty neat twist: Miles discovers that his universe was recreated, but was recreated with it's own Miles already in it.

Don't forget Jimmy Hudson, Ultimate Quicksilver, Ultimate Armor, Ultimate Mach-II, or that guy with wings. There's a lot of survivors.
