The connection between Mr. Sinister and Nanny was established in X-FACTOR (1986 1st Series) #40. Nanny "saved" Peter/the Orphan Maker from Mr. Sinister so he wouldn't be one of the Marauders. Written by Louise Simonson. Art by Rob Liefeld.

I found Nanny and the Orphan Maker as pretty twisted, even for a late 1980s Marvel comic. I suspect that was the point.
