While it's a nice page and a moment of bad-ass, the rest of the comic reminds us why he no longer has the title Sorcerer Supreme. Crimson Bands, doc, Crimson Bands....

His hands are capable of fairly regular tasks, but not the finest motor precision to put him in the top surgical class. They also look jacked up as all hell (which he usually hides with illusions), but they're supposed to be functional for most things.

Or were you talking about the World War Hulk thing? We can assume he healed them some time after (possibly with more dark magic before giving up his mantle).

We can't assume that, actually, given that his hands were still in bad enough shape during the recent Strange mini (final issue only three weeks ago!) that he was unable to do even the most basic spells unassisted.

On that, I'd blame Marvel's problematic inter-continuity between titles (where characters are regularly where they shouldn't be or not acknowledging things of paramount importance in other titles).

Strange's spell levels are diminished, but the consequences of that are inconsistent between writers.

I still think it's ridiculous that the skills of someone with that level of training and talent are supposed to be so hampered by his hands being broken. (Not to mention the fact that the Hulk broke his hands in a mindscape, which shouldn't have affected Strange's body at all!) I could see the loss of confidence from recent failures messing him up - it's done so before - but physical injury short of brain damage shouldn't slow him down with regard to magic.

Stephen's hands are always messed up, as is Tony's heart (sometimes his nervous system), Charles' legs, and Scott's eyes. Anything that changes this never sticks for long.
