The first time she actually died was when IIRC Logan killed her to spare her a painful death and instead it activated the full power of the Phoenix.
The second time was shortly thereafter when Mag-not-o (I've given up trying to work out who the hell that was after the retcons) killed her with an EMP pulse to the brain.

In 2000's? 30+ easy. Jesus Christ, Marvel. Do we always need moar new characters? It seems every writer insists on introducing a new x-man. And, while some time it really works out, I just think that the overall roster to date is getting a bit long.
