Mr. Sinister had a couple of backup plans in place in the event someone managed to successfully kill him.
She is a female clone of Sinister with his powers and his intelligence. But she doesn't have his memories or his accrued knowledge.
She was supposed to just be a vessel for his own consciousness to take over after he died, but she managed to avoid that and he found other clones to carry on through and now they both exist.
I don't think he actually managed to take her over. He was in the process of it but she fought against him, tried to switch bodies with X-23 (which obviously failed) to avoid it, and then somehow he got redirected into an empty clone body of a girl named Alice while Miss Sinister was unconscious.
I think Mister Sinister next showed up with that whole Sinister London schtick where he had an entire city populated by clones of himself but Miss Sinister hadn't appeared anywhere until X-Men Blue
After first reading this two thoughts immediately jumped into my head. One was "Alex is with the obvious bad guys. Something's wrong." And the other was of course "Has the Call Me Alex thing been working out?" I don't know why, that's literally what happened in my head.
Well, he has kind of evolved his position. Now there are acceptable setups with mutants in charge and unacceptable ones. The latter seems to involve large amounts of unnecessary mutant or human suffering.
Of course, what he'll accept probably varies with the desperation of the mutant position.
This is a man who saw medical experiments being forcibly carried out on his people during his childhood. I think he might be a bit -- touchy on that subject.
I have to say I'm a little curious if you're talking about Alex or Emma. Putting aside the recent nonsensical push to make them both villains, they both tend to make some questionable decisions.
Personally I see Alex as someone who tends to do a lot of stupid things with good intentions and Emma as someone who does morally dubious things for the greater good.

Like, if Emma's on team bad, I expect it's because she's up to something that'll help the good side. If Alex is on team bad, I expect it's because he did something dumb he'll regret later.
The outfits for g for both Emma and Ms. Sinister are so... questionable? Also after all the work with Emma, we get this. I don't even mind her being on the bad guys side, but she's the sidekick to Alex?

Like, you're not even going to try to answer his objections other than to say 'you'll have to get past it'? Like, was Alex inverted during Axis? And no one ever noticed since then? Poor Havok, always the Summers brother to get kicked in the gut for decades.
As stupid as I think Alex's team is, I have to say it's more interesting than Lorna's. I mean she's working with Bloodstorm (but not the established Mutant X one for some reason) and a bunch of characters most I either dislike* or have never heard of. This just screams "placeholder team until next relaunch".
*I mean they even picked the two worst Wolverines.
