Cold War's a crossover between the two currently running Cap books, Symbol of Truth and Sentinel of Liberty.
It weaves their respective plots - Sam Wilson contending with the work of the White Wolf and Steve Rogers grappling with the Century Game - together with a team-up of the Wolf and Bucky Barnes (almost certainly spun out of MCU!Bucky having " White Wolf " among his nicknames).
In Captain America: Symbol of Truth #11, the White Wolf's schemes had come to an end - he'd fought Sam Wilson and been defeated, taken into the custody of the International Criminal Court.
" You know, Bucky Barnes..
" .. we're very much the same, you and I.

" I'm a king.
" And you're a pawn. "

" .. I'm holding all the knives! "
(Bucky's new morphic arm comes from Captain America: Sentinel of Liberty #6 - it's a replacement for the one cast off when he broke away from Steve to join the Outer Circle playing the Century Game, to fight them on their own level.)
The Wolf and the Century Game's Revolution went knives to arm.

(" The transmitter " is technology the White Wolf used to command the works of Arnim Zola.)

The thread from Symbol of Truth #11 having been picked up, the narrative switched over to the one from the end of Sentinel of Liberty #11 - Ian Rogers reuniting with Steve Rogers and Sharon Carter.
Ian and Steve were catching up over breakfast at a diner when Steve noticed something: " Ian. "

The trigger on the weapon sighting them was pulled.
There was an explosion, and Steve staggering out of the aftermath.
" I really am sorry it had to be like this, Steven. "

Peggy Carter exited with Ian.
Steve stood, his adoptive son kidnapped, stung by a second betrayal - and called Sam Wilson, whom Ian'd been working with.
Four hours, Steve and Sharon were meeting with Sam and Misty Knight.
(Misty's been a supporting character in Symbol of Truth, on the periphery of the action.)
Together, they summed up everything that'd happened so far, up to Ian's kidnapping.

(David's a civilian-life friend Steve made over the course of Sentinel of Liberty #1.
Zola went off the table in last year's Captain America #0, which started Sentinel of Liberty and Symbol of Truth.)

" .. unless you're willing to play the game. "
(The crack in the table is something Steve pounded in a moment of frustation.
The skull in the table is what Sharon was carving out, evidently subconsciously.)
Elsewhere, Bucky and the White Wolf took delivery of Ian from Peggy.
They drew blood from him, restrained him, and gave him an electric shock.
Bucky followed that last up verbally: " Ian, I've afraid I've got some bad news.

" And now Dimension Z is mine. "
(Dimension Z being a place of Arnim Zola's, and Ian being a biological son of Arnim Zola's, the implication is that the latter was given the " keys " to the former as some kind of inheritance or insurance policy.)

As the White Wolf declared war, the Outer Circle was playing war in Alaska to train up their forces.
Steve, Sam, Misty, and Sharon dropped into the midst of that.
The war game changed, from " Outer Circle red team versus Outer Circle yellow team " to " Outer Circle army versus the intruders ".

(It's not stated how Steve knows the Power and the Money of the Outer Circle are behind this war game - presumably, his conclusion's partially rooted in one side being red and red being the Power's color scheme.
The other's in yellow, and per the hologram-to-face meeting he had with them in Sentinel of Liberty #5, yellow was the color of the Money and Bucky's predecessor as the Revolution - and the latter being dead, " yellow team " meaning the former would most likely fellow.
Either all of that, or he's derived that from the decoding of the Century Game's " move broadcasts " he and some friends worked out in Sentinel of Liberty #1. Said decoding was mentioned to still be effective in Captain America and the Winter Soldier Special #1.)
The battle was interrupted by the White Wolf and Bucky's forces.

The issue ended on Bucky and the White Wolf.

I remain wildly unimpressed with the whole deep-state conspiracy-theory idea of the Outer Circle (the enemy was your character design ALLLL ALONGGGG, STEVE!), which feels less like an attempt to address the powerful who manipulate events and more like a dodge. I thought at least Peggy, who had a great deal of goodwill left over from the movies and her TV show, was going to be too sharp to end up part of this mess. Guess that's a no.
The only thing about it that feels relevant is that in the current age of disinformation, I too have seen good people who should know better commit themselves to total bullshit. But I'm not gonna keep carping about the plot after this--it'll probably be easier to just wait it out.
