It definitely is gratuitous (and to be honest I think this comic was a bit of a nonsensical mess overall) but imo it's also a testament to how iconic the original TKJ is.
Even Gotham paid homage to the panel of Joker clutching his head:
My guess is Joker's gone through some kind of rebirth/gotten a new lease on life, and they're using that scene from The Killing Joke to represent as much (see also the flatline descending into his laughter). Being left to die by Batman and managing to outlive him would surely leave a mark.
I quite liked this series! Spooky, haunting, and a gorgeous showcase of the Dark side of the DC Universe and Lee Bermejo's talents. I'd love to see how a Brian Azzarello-penned Justice League Dark would turn out.
I still don't get why the literal Batdickery was enough to shuffle the whole label around, though, when other DC comics have shown nude women or even full on sex and gotten away scott free. Heck, blood, gore and demonic imagery is fine, but a penis isn't?
Pure cowardice by management, it was borderline invisible in shadow and and people freaked out like they haven't been doing porn parodies of batman for 50 years. I enjoyed this series but I want to see what the original plans were.
I think it was a PR thing. When news of the new Black Label imprint and Damned first broke, like ALL the online buzz was about Batman's penis. So I could see DC backtracking because they didn't want people thinking of Black Label as "the penis imprint."

I agree, especially about the art--it's gorgeous and terrifying. Everything from the seance panels to the cracked-mask creature to the pitch-black shadow of the Bat advancing in the alleys to the way faces are illuminated is freaky.
